Exploding foamed concrete
In September 2009 an Interim Advice Note was issued by the Highways Agency.
The Health and Safety Executive (HSE) has identified several incidents involving foamed concrete, including a serious accident at a sewage pumping station being decommissioned, when contractors were injured as a result of a gas explosion.
Blind bolt failure on offshore platform
A reporter describes how there was a failure on an offshore platform of a 20 tonne winch base under load testing. No injuries, but significant business impact. Failure occurred in tension of a holding down arrangement of blind bolts which were of the locking pin/cam variety.

The database contains all the CROSS reports that have been published and SCOSS reports since 1999. There is a Quick Search facility and any word entered here will be used to find all the reports which contain the word. There is a more refined search engine which looks for categories entered into a series of tick boxes.
'Structural safety' refers to the strength, stability and integrity of a structure to withstand the conditions that are likely to be encountered during its lifetime. Structural safety is achieved through the proper procurement, design, construction and maintenance of the structure and the application of best practice.
